Use this service to tell us about any reasonable adjustments you need to make your contact with the surgery more accessible.
It is a legal requirement that healthcare providers make reasonable adjustments where required, to make sure health services are accessible to everyone. A digital ‘flag’ can be put on your medical record to notify all care organisations of the adjustments you require.
Reasonable adjustments can be things like:
- making sure there is good access for people who use a wheelchair
- providing plain English, easy read appointment letters or resources in languages other than English
- offering a longer appointment or using a communication chart to make sure patients have the time and support they need to understand the information they are given
A digital flag can be put on your record if your impairment has a ‘substantial and long-term adverse effect on normal day to day activity’. If you feel that you have needs that meet this requirement, please let us know.
We may then contact you to discuss in further detail before adding any information to your record.
